Chin surgery, also called chin augmentation, is a commonly performed surgical procedure that enhances the shape and projection of the chin to improve appearance. The majority of patients seeking consultation for chin surgery desire an increased projection of the chin and a chin implant is effective in achieving their goal for improved appearance. Occasionally chin surgery is performed to reduce the projection of bone when the chin is too prominent. In select patients who have very small or asymmetric lower jaws and chins, surgery on the bone of the lower jaw may be needed to achieve the desired change in appearance. Dr. Derderian did a one-year fellowship in craniofacial surgery at the University of Pennsylvania that focused on reconstruction of the skull and facial bones to improve appearance. For the vast majority of patients, a simple chin augmentation with an implant will effectively achieve their goals for an improved appearance. Dr. Derderian typically uses a silastic implant for this purpose. The implant is placed through a small incision under the chin during a short outpatient procedure.

The benefit of Dr. Derderian’s craniofacial experience is his ability to identify those patients who would not benefit from a chin implant and would be better served with a genioplasty procedure. Genioplasty is a procedure when a cut is made in the bone of the chin and the bone is repositioned to correct an overly prominent chin, asymmetric chin or severely underdeveloped chin. It is very common that patients seeking rhinoplasty would also benefit from having a chin augmentation in order to achieve better facial proportions.  An underdeveloped chin magnifies the perceived size of the nose and improving projection of the chin dramatically magnifies the improvement from rhinoplasty for these patients. The best candidates for chin augmentation are physically and psychologically healthy and do not smoke.  Having realistic expectations for what can be achieved from chin surgery is important.

What Happens at the Consultation?

It is important that the patient has a clear idea of what improvements they want for their appearance and have realistic expectations for what can be achieved with chin surgery. Dr. Derderian will perform a detailed history and examination with special attention paid the jaw relationships. He will discuss your specific goals in detail at your consultation and will develop a surgical plan tailored to your anatomy and goals. It is common to get standardized x-rays (cephalograms) if there is concern about an asymmetric or underdeveloped jaw.

How is the Surgery Performed?

For most patients a silastic chin implant will achieve their goals for improved appearance. The appropriately sized and shaped silicone implant is placed just above the bone through a small incision under the chin.

In a small group of patients, the bone of the chin is underdeveloped or asymmetric and osteotomy (bone cut) is made through an incision between the lip and gums. The bone is move to the predetermined position and fixed in position. This procedure is extremely powerful and has a dramatic effect on the appearance of the profile due to reshaping of both the chin and neck line.

What is the Recovery Period?

Chin implantation is a brief procedure. The patient may require prescribed pain medicine for 1-2 days. Patients return to school or work in 7-10 days. Most of the swelling resolved by 10-14 days after surgery.

When is the Final Result Appreciable?

The effects of chin surgery are immediately appreciated. Even though swelling is present for 10-14 days, most patients feel comfortable in public after just a few days.
